POSITION SUMMARY

            To provide supported living and attendant care services for adults with developmental disabilities at home or in community settings. Position requires flexible hours and on-call availability. Position may require some weekends and evenings. Position requires a driver's license and own vehicle.

DUTIES AND RESPONSIBILITIES:

  1. Essential Functions
  • Transport clients and assist with grocery shopping and other errands and appointments.
  • Assist with medical appointments, including scheduling and attending, understanding diagnosis and treatment instructions, and getting referrals.
  • Help with ordering prescription refills, understanding medication instructions, picking up refills, filling med boxes, and scheduling medication reminders.
  • Help with accessing community resources, such as food bank, HACSA/Section 8, OHP, family support, and parenting classes.
  • Other assistance as needed, such as applying for food stamps and government assistance, filling out paperwork, searching for housing options, and GED classes.
  • Provide support for community inclusion, including hobbies, exercise, art and music classes, social groups, outings, and other preferred activities.
  • Provide hands-on assistance and reminders with household chores and responsibilities, such ascooking, laundry, dishes, cleaning the kitchen and bathroom, sorting, and donating or throwing away old items.
  • Assist with activities of daily living such as hygiene and personal care. Physical support as needed (transfer, ambulation, or standby assist).

B. Initial Training

  • New employees will receive an orientation to learn about the company and specifics of the job.

You will be introduced to clients and will be provided with training and support to ensure proficiency in your essential job duties.

C. Case Management

  • Document and maintain data on client activities, goals, progress and challenges.
  • Maintain all necessary records.
  • Track personal time and travel.

 D. Non-Essential Functions

  • May be required to cross-train for job coaching and be a backup as needed for job coaching shifts.
  • Attend conferences, workshops, in-services, and make presentations of information.
  • Participate on interviewing team as necessary.
  • Ensure safety requirements are practiced as outlined in safety manual, and that fire safety training occurs regularly.
  • Perform additional job duties as assigned.

 

QUALIFICATIONS

  1. A. Education and Experience
  • High School Diploma or equivalent.
  • Experience working with developmentally disabled adults.
  1. B. Knowledge, Skills, Abilities and Licenses
  • Behavior management/goal setting.
  • Data collection/report writing.
  • Problem solving.
  • Must be able to work independently for long periods of time.
  • Ability to advocate for the needs of clients in this program.
  • Ability to interact with clients with challenging behaviors, various types and severity of disabilities, and clients from different backgrounds.
  • Valid Oregon Driver's License.
  • Must pass a DHS background check.
  1. Contacts
  • Regularly scheduled client contact as outlined in the support plan.
  • Contact with support agencies as required.
  • Contact with supervisor and other PBC support staff as required.

 

REQUIREMENTS

  1. Movement
    • Must be able to travel to various locations meeting time commitments.
    • Must be able to lift up to 50 lbs. on occasion.
    • Must be able to negotiate stairs independently.
  2. Communication
    • Must be able to communicate with staff, clients, family members, and government agencies.
    • Must be able to use electronic devices for email, documentation and report writing
  3. Sight & Sound
  • Must be able to observe, inspect and identify hazards in the workplace, the community and clients' homes.
  • Must be able to converse and exchange information with others to respond to issues related to client needs.


4. Environment

  • Work in different environments subject to various conditions, including clients' homes, workplaces, and public locations.
  • Travel and activities require exposure to outdoor conditions.
